NHS England is reviewing DCB 0129 and DCB 0160, the two clinical risk management standards mandatory across health and social care. I authored them, and this is my full response to that review, published openly rather than left in a survey field.
My position is short: keep the standards, and modernise the model beneath them, from a one-off safety case signed at go-live to continuous, through-life assurance.
